Tokentool is very useable as it is.
It would be nice to be able to try different backgrounds behind the token. Perhaps start with the default backgrounds for unbounded maps in maptool.
It would be nice to get rid of the magenta border from the leftbar.
It would be nice to be able to launch it from a menu in maptool.
The image size that comes out ought to be a configuration option, not dependent on the size of the template -- and get rid of redundant templates. We can give a hint as to the best resolution to save a template at to get best results at various output token sizes.
only thought I had

Backgrounds
Not sure what you mean, could you ellaborate which background you are talking about ?
Border from left
True. My original thought was that it would be useful to be able to see what was going to be transparent in the final token, but I think that's fairly obvious because of the preview window. I'll update that.
Menu in maptool
Hmmm, interesting idea. A challenge would be to figure out whether to try to webstart it or try to find it locally (in case someone has downloaded the zip)
Image size
I have been thinking this exact same thing
Soon as MapTool goes 1.0 I'll give TokenTool some TLC
