Out of game rules

After a year of being inactive I'm looking to start DMing again soon. Since I had bad experiences with online play (e.g. people drowsing the web when it wasn't their turn), I've decided to play face-to-face again. I want to avoid some of these interpersonal pitfalls by creating some clear agreements before starting play.

I'm hoping to draw upon your collective experience for topics that you think should be decided upon before starting a game.
So far I have divided the ones I've come up with in two categories:
- Out-of-game topics (e.g Use of alcohol, attendance, validity of dice rolls)
- In-game topics (e.g. Character death, metagaming, PvP)

I'm not looking for opinions on how to fill in those rules, just topics that should be decided upon before play.
Any advice you guys can offer would be greatly appreciated.

Alcohol - mandatory, obviously.
