Already, a token can be moved in the Properties->Config->Layout window to adjust its anchor point (it'll always rotate around the point that's in the center of that window). I think, since 4e is so decidedly square-centric, you might allow a separate, simpler vision system entirely based on squares and their corners. Actually, after setting an image to freesize you can drag the little box in the bottom right corner of the image to resize it however you want.
